COVID-19 Notice: Live Scan appointments are currently available for limited positions. All customers of Live Scan at the UC Davis Police Department must adhere to public health safety precautions. Please reschedule your appointment if you have experienced any cold or flu-like symptoms within the last 14 days. Space in the lobby is limited; please do not bring family members with you. You must maintain physical distance from others and wear a face cover.
If you are exhibiting symptoms or unable to follow these guidelines: Police Department personnel have the discretion to cancel or delay your appointment for at least 14 days.

Appointments are now scheduled with our online calendaring system (see Schedule an appointment), and are available Monday-Friday at the UC Davis campus, and the UC Davis Medical Center in Sacramento (see Directions).

Required information for your appointment

  1. Must present one of the following primary forms of photo ID:
    • California Driver's License
    • Department of Motor Vehicles Identification Card
    • Out-of-state driver's license
    If you do not have one of the primary forms of photo ID, check the list of acceptable secondary forms of ID and supplemental documents.
  2. Must bring with you the completedAuthorization to Release Information form, and the Fingerprint Authorization form, both provided to you by your hiring department.
  3. Clients must arrive on time to avoid rescheduling.

Appointments for Registered Nurses (performed at UCDMC only):

  • Updated Live Scan information (CA Board of Registered Nursing website)
  • Request for Live Scan form (CA Board of Registered Nursing website)
  • Once received, print and fill-out the Live Scan form
  • Check or money order for $46.80, payable to 'The UC Regents'
  • Valid government-issued photo ID (state-issued driver's license, government-issued ID or passport)
